Limits of the Out-of-Pocket Maximum

For many cancer patients in the United States, the out-of-pocket maximum provides important financial protection. It limits what a patient pays for covered, in-network care.

But it does not guarantee access to every treatment the patient may need. The limit generally does not include:

  • Out-of-network treatment
  • Services not covered by the insurance plan
  • Charges above the insurer’s allowed amount

These costs can remain the patient’s responsibility even after the out-of-pocket maximum has been reached. The patient may then face a choice between paying the full cost, delaying treatment, or looking for another treatment pathway.

A separate hospital review can proceed while insurance issues are being resolved. The practical questions are whether another clinically appropriate option is available, how soon it can begin, and what the total cost would be.
