APIS: International Medical Coordination for Proton and Carbon Ion Therapy

When Particle Therapy May Not Be Suitable

Particle therapy may be considered in selected cancer cases, but it is not suitable for every patient.

It is a local radiation treatment. This means its role is usually focused on a defined tumor area, not on controlling widespread disease throughout the body.

Particle therapy may be less appropriate in situations such as:

  • Widespread metastatic disease requiring systemic treatment
  • Rapid clinical deterioration or urgent need for local emergency care
  • Poor performance status or inability to tolerate travel and treatment
  • Severe organ dysfunction that makes treatment unsafe
  • Lack of a clearly defined tumor area for radiation treatment
  • Cases where prior radiation dose makes further radiation too risky
  • Situations where surgery, systemic therapy, conventional radiation, or supportive care may be more appropriate

In some advanced cancer cases, the main treatment priority may be systemic therapy, symptom control, or urgent care near the patient’s home.

For international patients, travel itself can become a medical burden if the patient’s condition is unstable. Hospital review, treatment scheduling, visa preparation, travel, accommodation, and follow-up planning all require time and physical capacity.

Particle therapy should also not be viewed as a last-minute solution when the disease is rapidly progressing and the patient is medically unfit for travel.

Before considering treatment abroad, it is important to understand the patient’s current disease status, recent imaging findings, performance status, organ function, prior treatment history, and immediate care needs.

In some cases, continuing care near the patient’s home may be safer and more practical than arranging international treatment.

APIS helps coordinate preliminary communication and document submission to Korean hospitals for selected international cases.

* Final treatment decisions are determined solely by the treating hospitals and physicians.
