APIS: International Medical Coordination for Proton and Carbon Ion Therapy

When Insurance Denial Changes the Treatment Timeline

For some cancer patients, insurance denial is not only a financial issue. In certain situations, it may also affect treatment timing.

Advanced cancers can continue to progress while patients and families wait for prior authorization, appeals, network approvals, or additional administrative review.

Published studies in proton therapy have reported that insurance denial and appeal processes can add weeks to treatment decision timelines. In some cases, delays of several weeks may influence whether local treatment options remain feasible.

This situation may become more complicated in cases involving:

  • Re-irradiation after prior radiation therapy
  • Tumors located close to critical organs
  • Locally advanced or unresectable disease
  • Limited local treatment availability
  • Situations where multidisciplinary review is required

Some patients pursue additional reviews, including international case review, while insurance appeals are still ongoing. This does not necessarily mean overseas treatment is appropriate or recommended. In many cases, local treatment remains the most practical option.

However, some patients and physicians may wish to understand all potentially available pathways before treatment eligibility changes further.

International review may require:

  • Recent imaging in DICOM format
  • Pathology reports
  • Previous treatment history
  • Recent blood tests and current performance status

Whether treatment abroad is appropriate depends on disease status, travel fitness, timing, cost, and hospital eligibility review.

APIS coordinates preliminary communication and case review requests with Korean hospitals for selected international patients seeking information regarding particle therapy options in Korea.

* Final treatment decisions are determined solely by the treating hospitals and physicians.
